2009-10 Registration for Online Services
Please complete ALL required fields (marked with *) on this form. All information will be kept strictly confidential for use only in creating personalized, password-protected family accounts. If you also would like to be added to the CPAPO email distribution list for weekly updates on activities and events, please check the box where indicated. If you do not have access to email, please contact the lunch coordinator.
IMPORTANT! Each student may be registered under only ONE (1) family account. If access to a student’s account is required by more than one household, the account username and password must be shared.
Once your account has been created and activated, you will receive an email with the username, password, and a link to the site. If desired, passwords may be changed after you are logged in. Accounts are activated upon payment of the registration fee ($10 per student), which covers the fees associated with maintenance of the online services offered by the PO. Please note, the Hot Lunch program is a service which is only available online.
Families are strongly encouraged to access this form online here at the CPAPO web site in order to expedite receipt of their accounts. Use of the paper form may delay your account creation by up to a week, due to the required manual data entry. The online form includes both a Submit function and a Summary Page so families can print a copy of their registration, attach payment in the form of cash or check, and turn in to the PO mailbox at the school’s front desk.
